Welcome to the Ontario Soccer Organization Provisioning form. This form is required for all Ontario Soccer Member organizations to gain access to and submit registration data into OSCAR. All Districts, Academies and Clubs will need to provide updated information related to their organization. This information will be used to begin the provisioning process of each organization.
*Please note that provisioning takes place once per week on Thursday.
